I am struggling with an electric linear actuator, it has a reduction internally.

There is a metal worm gear and a the worm-wheel is made of plastic. The worm-wheel is wearing out in some teeth (at one point), it looks like combination of high temperature and overload.  

The actuator runs at ambient temperature, there is no overload (the actuator has more then enough capacity 4000N) is running within the min and max amperage as prescribed by supplier, it has a software protection as well. 
 

Any suggestion where to start looking for ?
